Most macros in unlocked-io.h had the wrong number of arguments.
* gen-uio: New script.
* unlocked-io.hin: Remove file.
* Makefile.am (unlocked-io.h): Rewrite to use a separate script,
rather than trying to embed it here.
(EXTRA_DIST): Add gen-uio. Remove unlocked-io.hin
